If you wish to make a claim of loss or damage of a package, you must submit a completed claim request form to GSO within 6 months of the shipment pickup date. Claim forms are available through the Customer Service Center.
To determine the cost of a lost or damaged item, you will be required to provide original purchase invoices or estimates for repair, and may be asked to provide copies of your original shipping forms.
Damaged items must be made available for inspection, including all package contents, original shipping material, packing material, and shipping carton.
